Proof of residency is a current utility bill (water, gas, phone, etc) or a lease agreement or mortgage statement with the parent/guardian name and current address. Please bring your complete packet and proof of residency to the GEMS Front Office (8:00am-4:45pm) or e-mail the information to Sarah Salvatore at g.
